GSS Premium Debt and Income Test Inquiry

Everyone residing in Turkey, if they don’t have an insured job, is automatically included in General Health Insurance (GSS) scope. This situation concerns millions of citizens who fall into debt without knowing.

1. Is GSS Mandatory?

Yes. From 1.1.2012, everyone without social security (unemployed, young graduates) is obliged to pay GSS.

• Students: Benefit from family’s insurance until 25 (or graduation), do not pay premium.
• Unemployed: Premium is paid by state as long as they receive unemployment allowance.

2. What is Income Test?

If you don’t have power to pay premium, you can go to District Governorship Social Assistance Foundation in your residence and have “Income Test” done.

• Green Card (G0): If income per capita in household is less than 1/3 of Gross Minimum Wage; State pays your premiums (Formerly Green Card).
• Paid (G1): If income is above this limit, you have to pay monthly standard GSS premium. (For 2025 this amount is around 600-900 TL band).

3. What Happens If GSS Debt Is Not Paid?

GSS premiums are also a “Public Receivable”. If not paid:

1. Interest Accrues: Delay penalty is added.
2. Health Service is Cut: If debt exceeds 60 days, you cannot receive “Provision” in state hospitals (Except emergencies).
3. Execution Proceeding: Theoretically SGK can do execution proceeding (However in practice usually amnesty/restructuring is waited).

4. Erasure of Debt and Restructuring

State erases interests of GSS debts or installs the principal by issuing “Omnibus Law” at certain intervals. It is important to apply when such law is passed.

Q: University finished, does debt occur immediately? A: Yes. GSS debt starts to accrue from the date of graduation (if you are under 25 and male, or if there is no right from mother/father).

Q: Is retroactive income test done? A: Normally cannot be done. However when amnesty laws are passed, sometimes rights like “6 months period for those who never had test” can be granted.
